Concierge primary care is a membership-based healthcare model that offers direct access to your physician, longer appointments, and highly personalized care. The focus is on prevention, wellness, and ongoing communication, rather than rushed, episodic visits.

Membership is $5,400 annually for individuals. For households with two or more adults, membership is $4,860 annually per member. Membership is designed to be simple and transparent, with annual billing.

Your membership covers non–insurance-covered services, including extended visits, expedited scheduling, enhanced care coordination, and your annual Functional Fitness and Performance Physical. Medicare-covered and commercially insured services are not included in the membership and will be billed separately when applicable.

This comprehensive evaluation provides a detailed assessment of your overall health and physical performance. It includes body composition analysis, caloric needs assessment, strength and functional fitness testing, mobility evaluation, and optional advanced fitness services.

In addition to primary care, members have access to executive coaching and advanced wellness and fitness services. These are optional add-on services. Please contact us to learn more and schedule a consultation.

Yes. We are in-network with most major insurance plans, and insurance is used for covered medical services such as sick visits and annual physicals. Membership fees cover non-covered services.

If your plan appears out-of-network on platforms like Zocdoc, it may be due to variations within insurance plans. Please call us directly and we're happy to verify your coverage.

Members may schedule appointments via direct messaging, phone, or our online portal. Prospective members are asked to complete a complimentary meet-and-greet visit before scheduling wellness or problem-based visits.

This complimentary visit allows prospective members to meet our physicians, learn about the practice, and ensure Bespoke Primary Care is the right fit before enrolling.

Absolutely. We coordinate closely with specialists, imaging centers, and laboratories to ensure seamless, well-organized care. Referrals and follow-up are actively managed on your behalf.

We offer extended visits, direct physician access, personalized wellness and fitness planning, and a strong emphasis on prevention. Our patient panel is intentionally limited to ensure thoughtful, unhurried care.
